Start the live-pricing refresher for a standalone command that owns the runtime without a daemon (zeroclaw gateway, zeroclaw channel start).

The daemon does not use this: it starts the refresher on its generation’s shared live configuration. A standalone command has no such handle, so this binds a copy of config; the standalone gateway then re-binds the refresher to the live handle its config API writes. The refresher starts at most once per process and is a no-op unless a provider sets live_pricing = true.
