Point the refresher at the live config handle a surface writes, without starting it.

Starting the refresher belongs to the process that owns the runtime (the daemon, or a standalone command). A surface that owns a live config handle, one its config API writes in place, binds that handle here so the next cycle sees an operator’s change (an opt-out, a new endpoint or model) without waiting for a reload. The gateway does this at startup.
